Change a Life Foundation Scholarship

Scholarship Criteria:

1. Must be attending a California public high school as a 12th grade student and enrolling in a four-year College or University –OR- Must be attending a Community College in California and ready to transfer to a four-year College or University.
2. Must have a minimum weighted GPA of 2.75 from 9th grade to the first semester of 12th grade –OR- cumulative GPA of 2.5 for community college courses.
3. Must have a minimum SAT score of 1350 or minimum ACT score of 19 (if applying as a high school senior).
4. Must be economically disadvantaged
5. Must be a U.S. Citizen or Legal Resident.
6. Must show evidence of extracurricular activities such as school activities, community involvement, and/or employment.
7. Must have community service.
8. Must have above average attendance.

Eligible applicants may apply for this scholarship by completing an online application at: www.changealife.org/how_to_apply/default.aspx

In addition, applicants must submit (via fax or email):

1. SAT or ACT score verification
2. Transcript of grades
3. Two letters of recommendation from a teacher, college/guidance counselor, or mentor
4. Copy of your college acceptance letters to the institutions you were accepted
5. Copy of W2 tax forms for income verification
6. Photograph of yourself
7. Two essay questions
