How to Apply for a Permit

CHILD DEVELOPMENT PERMIT APPLICATIONS
HOW TO OBTAIN FORMS

YOU WILL NEED TO VISIT TWO DIFFERENT WEBSITES IN ORDER TO ACCESS ALL OF THE FORMS NECESSARY FOR A COMPLETE APPLICATION PACKET.

FIRST TIME APPLICANTS

FIRST: Child Development Permit Applications are available by either visiting the Child Development Training Consortium Website or calling 1-209-572-6080.
You will need the following forms:

  1. CDTC Child Development Permit Funding Application.
  2. Verification of Experience Form
  3. Application for Reimbursement of Live Scan Fingerprint Processing Fees
  4. Confidential Profile for Direct Service Participants

TO ACCESS FORMS ONLINE:

Visit http://www.childdevelopment.org
Click on the SERVICES tab and find the green box to the left of the screen. Click on the heading CHILD DEVELOPMENT PERMITS and find the yellow box to the right of the screen title CENTER BASED/GENERAL CHILDCARE APPLICANT. The first item listed is the General Childcare-First Time PDF Packet. This packet includes the CDTC Child Development Permit Funding Application, Verification of Experience Form, the Application for Reimbursement of Live Scan Fingerprint Processing Fees form, and Confidential Profile for Direct Service Participants.

SECOND: Two other forms are available from the State of California Commission on Teacher Credentialing Website, by calling 1-888-921-2682, or by making an E-mail request from credentials@ctc.ca.gov.

You will need the following forms:

  1. Application for Credential Authorizing Public School Service (Form 41-4)
  2. Request for Live Scan Service (Form 41-LS)

Under the blue CA EDUCATORS tab, click on the title HOW TO APPLY. Both forms are accessible from there.

RENEWAL APPLICANTS


Follow FIRST step but access the General Childcare-Renewal Assistant & Associate OR General Childcare-Online Renewal Teacher forms from the Child Development Training Consortium. This packet includes the CDTC Child Development Permit Funding Application and Confidential Profile for Direct Service Participants.
Follow SECOND step but access Renewal and Reissuance Application (Form 41-REN) from the State of California Commission on Teacher Credentialing.

UPGRADE APPLICANTS


Follow FIRST step but access the INSTRUCTIONS AND POLICIES/CENTER BASED/GENERAL CHILDCARE-UPGRADE form from the Child Development Training Consortium. This packet includes the CDTC Child Development Permit Funding Application, Verification of Experience Form, the Master Teacher Specialization Designation form, and Confidential Profile for Direct Service Participants.
Follow SECOND step to access the Application for Credential Authorizing Public School Service (Form 41-4) from the State of California Commission on Teacher Credentialing.

HELPFUL INFORMATION

  • The Child Development Training Consortium will pay the application and fingerprint processing fee (via reimbursement) for the following Child Development Permit levels:
  • Assistant (first-time and renewal)
  • Associate Teacher (first-time, upgrade and renewal)
  • Teacher (first-time, upgrade and renewal)
  • Upgrades from one of the three lower level permits to Master Teacher, Site Supervisor or Program Director
  • Permits are valid for 5 years.
  • Volunteer hours count under the experience requirement as long as you volunteer for a minimum of three (3) hours at a time.
  • Processing of permit applications can take up to three months once they are received by the Child Development Training Consortium office.
  • Effective September 1, 2008, the Commission on Teacher Credentialing will only provide permits through an online view and print process. The Commission will no longer print and mail these documents. Forty-eight hours prior to the posting of your permit document on the CTC website you will receive an email notification.

In regard to live scan fingerprinting:

  • The first time that you apply to the California Commission on Teacher Credentialing you must submit a Live Scan fingerprinting that is done specifically for the CCTC. No previous scans are accepted.
  • You do not need fingerprinting for permit upgrade or renewal.
  • Original receipts must be submitted in order to receive a reimbursement.
  • When you are at the live scan facility, remember to ask the live scan operator for a copy of the completed form for your records.
