How to Apply for a Permit
How to Obtain the Forms
First:
Child Development Permit Applications are available by either visiting the Child Development Training Consortium Website (www.childdevelopment.org) or calling 1-209-572-6080.
If you are employed working with children under the age of five, you will need the First 5 Child Development Permit Application.
If you are a volunteer or working with school-age children or in any other capacity, you will need the CDD Child Development Permit Application.
In both cases, you will also need the following forms:
- Verificaton of Experience Form
- Application for Reimbursement of Live Scan Fingerprint Processing Fees
- A form for Master Teacher Specialization Designation is also available.
To Access Forms Online go to www.childdevelopment.org/cs/cdtc/print/htdocs/services_permit.htm and find the box to the right of the screen titled Applications.
From there you can access both the First 5 Child Development Permit Application and the CDD Child Development Permit Application. Both of these applications are available in English or Spanish.
The Verification of Experience Form, the Application for Reimbursement of live Scan Fingerprint Processing Fees form, and the Master Teacher Specialization Designation form are all included in the online application packets.
Second:
Two other forms are required:
- Request for Live Scan Services Form
- Application for Credential Authorizing Public School Service (Form 41–4)
These two forms are available from the State of California Commission on Teacher Credentialing Website (www.ctc.ca.gov), by calling 1-888-921-2682, or by making an E-mail request from credentials@ctc.ca.gov.

Helpful Hint
When you are at the live scan facility, remember to ask the live scan operator for a copy of the completed form for your records.
