Initial Grant Proposal Form

To help you identify potential funding sources from state and federal agencies and private foundations and corporations, the Grants Office needs a completed Initial Grant Proposal Form outlining your project objectives, activities, and budget requirements. Once the completed form has been signed by your Division Dean or Department Head, we will help you write and edit your proposal narrative and budget.

The Initial Grant Proposal Form is available below in two formats: PDF fill-in form or Microsoft Word document. The PDF is easy to use and print; however, unless you have the full Adobe Acrobat application, you cannot save your input and return to it later. You must complete and print the form in one step. If you have the full Adobe Acrobat application, you may save the PDF form and return to it for additional input or revision as often as necessary.
