How to Report Change of Information

* Information includes: new address, phone number, e-mail address, major, Social Security number, and Tax Payer ID number.
* Changes must be reported within 10 days.

International Students who want to change information in their records need to:

1. Complete the Change of Information form found at L-104 counter.

2. Submit the form to L104 (you do not need to submit the form to
Records Office).

3. The International Student Office will change the information and
submit the form to the Records Office for you.

* Note: If you are changing your major, you need to turn in the form, an Educational Plan, and your I-20 to L104.
