Federal Employment Eligibility Form I-9: Use this form for to document that you are authorized to be employed in the United States.
W-4 Form: Complete Form W-4 so that your employer can withhold the correct amount of income tax from your pay.
LiveScan Form: Complete the Livescan form for finger printing services for your background check.
Student Workers And College Assistants: This file contains instructions that pertain to you as a student worker or College Assistant. It also provides links to websites for you to access additional important information.

Citizenship Verification: Submit this form to verify US citizenship or permanent resident status.
Appeal for Dependency Override: Use this form to appeal to have your dependency status changed from Dependent to Independent with special circumstances.
Eligible Program Certification Form: In order to qualify for federal or state financial aid, you must have a clear educational goal (transfer, AA, A.S. or certificate) at Pasadena City College. Once you decide on a goal, you also have to have a declared major. Use this form to state your educational goal.
Satisfactory Academic Progress Standards: Rules to meet satisfactory academic progress requirements and maintain financial aid eligibility at PCC.
Consortium Agreement: Students attending another academic institution concurrently must complete this form.

Federal Direct Loan Program Appeal Request: Use this form when you have been notified that Pasadena City College will not certify or has postponed certification of your Federal Direct Student Loan. You may explain circumstances for negative repayment activity such as a previous default or provide an explanation that substantiates why certification should be considered, e.g., demonstrated improvement in your academic performance at PCC.
