Congratulations on completing all the requirements to earn an Occupational Skills Certificate! To have your certificate issued, complete the following steps:

1. Complete the Application of Issuance of a Occupational Skills Certificate
   
   Visit your division office to request a copy of the applicaiton.
   
   
2. Submit your completed application to your academic division office
   
   
3. Division Dean Review
   
   Your Division Dean will review your application and transcripts and determine whether or not you have met the requirements for a certificate to be issued
