The Spring 2026 Semester Intake Form is no longer avaliable. Registration for the Second Half of the Spring semester courses, Math 005B and Math 055, will open on March 30 at 10:00am. The Fall 2026 Intake Form will be avaliable mid-May.

1. Watch the Orientation Video

You can watch this at anytime and as many times as you would like!

2. Complete the Orientation 

You will need to complete the Orientation Form with a score of 100%. 

3. Complete the Intake Form 

Once you have completed the Orientation form, you will be prompted to fill-out the Intake Form. Please note that completing the Orientation and Intake Form does not guarantee a spot in Math Path. After submitting the form, you will be emailed by the Math Path office within 48 hours about how to register for a Math Path course.
